Not nice to make the observer fill out this long a form at the end of a loong night.   
Conditions by QUARTER?  Why not SIXTEENTHS?

Getting to this form is itself a trial!  I counted having to click 8 times to get to the form...   I thought the rule of thumb for a webpage is 
less than or at atmost 4.
