Problems: 

Failure of mirror bag system causes jumps in tracking, leding to dramatic loss of guiding.  Huge excrusions in the DECam guide box;
trails and multiple images on frames.  Began as subtle effect with intermittent loss of guiding; progressed to much worse.
Night crew was able to recover but we were advised after 3rd occurrence that the telescope would have to be safed for the night;
this did nappen.

Comments: 

Guider problem as described above.  Occasional failure to set up guide stars in some fields.
